Prequalification Questionaire

Thank you for your interest in employment with our agency. Any offer of employment is contingent upon a negative drug test, a qualifying criminal background check, a qualifying motor vehicle report, and being eligible to participate in any federal contract program.

To proceed with the application process and be considered for employment, you must be able to answer 'YES' to the following statements. Please check 'YES' or 'NO'.
I am 18 years or older
I have graduated from high school or have a GED
I am eligible to be employed on a federal contract program, since I have never been debarred, suspended, or excluded from a federal program or government contract.
My professional license is current and valid. (It hasn't been revoked or suspended.) If not applicable to you, put N.A.
To be considered for employment, you must be able to answer "no" to the following. Please check "yes" or "no".
My driver's license is currently revoked or suspended.
I have a DUI conviction (Driving Under the Influence) that is 3 years old or less.
I have had a conviction of reckless driving that is 3 years old or less.
I have multiple moving vehicle violations that are 3 years old or less (2 or 3 motor vehicle violations and/or accidents; eligibility for our insurance is dependent on severity, circumstances, etc.).
To be considered for employment, you must be able to answer "no" to the following. Please check "yes" or "no".
I've had a felony conviction related to controlled substances (since 1996).
I've had a conviction of other felony crimes within the past 5 years.
I've been convicted of abuse, neglect, assault within the last 5 years.
If you answer, "yes" to the following section, you may still fill out an application.
However, depending on the crime, it may exclude you from consideration. Circumstances considered include number of convictions, length of time
since the conviction, and the severity of the crime.
I have been convicted of a misdemeanor crime in the last 3 years.
